NOSE AESTHETICS (RHINOPLASTY)
My aim in rhinoplasty is to make a natural and impressive nose suitable for the contours of the person instead of a nose that looks incompatible with the facial contours, which is uniform and obviously made. Since it is located in the center of the face, a correctly planned nose has a positive effect on the face.
While evaluating my patients, I look at the proportions, angles and profile image of the nose with the structures on the face and make a design in the computer environment. I also evaluate the forehead, cheekbone, lips, chin and jowl area and make a profile aesthetic plan. Afterwards, I definitely examine the inside of the nose and if there is a problem with breathing, I plan our treatment for it.
You will not have pain after surgery. Sometimes there may be tingling.
Surgery can be performed with open or closed techniques. I determine the method according to the patient's needs. General anesthesia is appropriate. If no bone-related procedure will be performed and the patient is compatible, it can also be performed with local anesthesia.
I do not break the bones in the nose, instead I cut or file the bones.
It is not necessary to put a tampon, but if a corrective procedure has been performed in the nose, it should definitely be placed. It is silicone and perforated tampon and does not disturb the patient much and is easy to remove because it does not stick when it is removed.
A protective splint is made on the nose after surgery. There are those who keep the splint for 5 days, but I prefer to keep it for 7 days, I don't want to risk it for 2 days. In the follow-up, I stick small bands for the swelling to go down quickly and regularly, I remove it after 4-5 days.
Do not believe what is said as a completely bruise-free and swelling-free surgery. It is natural to have swelling and bruising, but it resolves within 5-7 days depending on the person. In my patients, there is no swelling and bruising at all, and there are those who have it, it may vary depending on the person.
There may be slight leakage in the first 24 hours.
The swelling in the nose usually starts to go down after the 2nd week and takes its final shape in the 6th month at the earliest. It may last up to 1 year in some patients.
For profile aesthetics, if there is a backward opening in the forehead area, it can be supported with fillers. If the cheekbones are behind, they can be emphasized with fillers or prostheses. If the lips are thin, they can be thickened with fillers. If the chin is behind, prosthesis or filling, if it is ahead, bone reduction or regression can be performed. Liposuction can be performed for fat in the jowl area.
The best candidates for this procedure are patients who have well-defined expectations and who do not have very thick skin without psychological problems.
After surgery
Head should be lying high
Ice should be applied under the eyes (36 hours)
It is useful to use bruise removal creams.
Since the inside of the nose can be clogged with clots and secretions, it should be kept moist with sea water sprays, and clots can be removed with an ear swab dipped in oxygenated water. Do not blow your nose.
For nasal rest, meals that require excessive chewing should be avoided, excessive laughing and tooth brushing should be careful.
Sports should be avoided for at least 4 weeks.
Solarium and sunbathing should be avoided for 3 months.
Glasses should not be used for 3 months. Lens can be used after 3 days.
Head can be thrown back and hair can be washed.
Alcohol should not be taken for at least 2 weeks.
COMPLICATIONS AND POSSIBLE PROBLEMS
Bleeding, adhesion in the nose called synechiae, perforation in the structure that divides the nose into two parts called septum, infection. These are very rare and treatable problems.
The most important thing is aesthetically, that is, not liking the shape of the nose. Even in the best surgeons of the world, this rate is 10 percent.
NOSE TIP LIFT
Some patients are only bothered by the tip of the nose. A low nasal tip reduces the prominence of the upper lip and can add an aged appearance to the face. It is one of the most important points in rhinoplasty. It not only affects the appearance but also contributes to breathing as it improves the external valve mechanism of the nose. It is easily performed with local anesthesia and does not prevent the return to daily life. Since it is done from inside the nose, there is no trace from the outside. It does not use a splint, only tape is sufficient.
NOSE AESTHETICS WITHOUT SURGERY
It is a procedure in which fillers and botox are used to eliminate minor problems in the nose. It does not correct noses that require surgery. Irregularities in the root of the nose and the back of the nose can be eliminated, filling the nasal lip angle and lifting the tip of the nose, filling the tip of the nose and making an elegant appearance. Sometimes it can be used for postoperative irregularities. Botox can make a slight lift at the tip of the nose when it is applied to the muscle that pulls the tip of the nose down. But it should not be forgotten that these are temporary methods.
OPEN TECHNIQUE
It is generally preferred in noses requiring multiple procedures and revisions where a clear viewing angle is needed. The 3-4 mm scar under the nose becomes unnoticeable over time. I have never received any negative feedback from any of my patients due to the scar. The only disadvantage is that nasal tip swelling may last longer in some patients.
CLOSED TECHNIQUE
It can be used if the tip of the nose will not be treated too much. There is no visible scar from the outside.
NOSE WING AND HOLE AESTHETICS
They are structures that have both aesthetic and functional importance as they are the air intake route to the nose. It can be done alone or as part of rhinoplasty. The cartilage support of this region must be preserved. Otherwise, it cannot withstand the negative pressure of the inhaled air and collapse occurs and the amount of air entering decreases. The oval shape of the holes should be preserved. The size of the nostrils can be reduced or enlarged, the wings can be narrowed or thinned if they are fleshy.
